It remains a sad fact that accidents and near misses continue to occur during pilot transfers with frightening regularity. Most of these fortunately do not result in injury, and a surprisingly high number of them are not even recognised for what they are.

Surveys into pilot ladder safety consistently reveal that unacceptably high numbers of pilot transfer arrangements are not compliant with the regulations.

Reid of the Palm Beach Harbor Pilots Association on the nightshift, as he docks four different cargo ships of all different sizes that help support the islands of the supply chain of the Caribbean. Dredger OCEANLINE 5001 sailing at full speed struck anchored tanker STROVOLOS with moored alongside supply vessel, at around 1140 LT (UTC +8) Oct 2, on Melaka anchorage, Malaysia, Malacca Strait. The results can be seen on video – tanker suffered portside hull breach above waterline and apparently, some damage on cargo deck.
