Swedish Club: Pilot ladder in poor condition

It was a winter morning with calm winds and no swell. The vessel had departed port and was preparing for the pilot to disembark. The Chief Officer had told the bosun to prepare the ladder on the port side. The vessel had discharged all its cargo and was in ballast condition proceeding to the next port for loading. As the vessel had no cargo onboard the freeboard was about 7 meters.

When the bosun had secured the pilot ladder he called the Chief Officer on the radio and told him all was ready for the pilot.

A couple of days before, the safety Officer had done the monthly inspection of the safety equipment and had reported that the pilot ladder was in good, clean condition. The ladder had a four-year old certificate of conformity that was still valid.

I believe that safety officer inspected ladder visually instead of close looks.
He just completed the checklist as routine monthly inspection.

It has been exactly one year ago that a pilot from Sandy Hook (New York), Dennis Sherwood, died in the line of duty while attempting to board the container vessel Maersk Kensington.
What has happened since then? Has safety been improved for the pilots?

Article Skipper and pilot died in pilot boat collision, Kobe

Pilot boat with 5 people on board collided with breakwater at Kobe Port, Japan, at around 0300 Tokyo time Sep 4. 52-year old skipper of the boat, and 71-year old pilot, died in the collision, 3 crew were seriously injured.

Video Vessel "ELSE" crashes into closed lock gate in Kiel-Holtenau on 29.08.2020

Shortly after 5 a.m. local time the vessel “Else” had a collision in the Kiel-Holtenau lock to the Kiel Canal.
The "Else" subject to pilotage passed the pilot's boarding position without taking a sea pilot on board and collided from the outside into the closed gate of the new north lock. The bow bored into the metal construction and the ship was stuck for about six hours.
