Today i tagged along on the pilot boat Pacific Pathfinder and her sister ship the Pacific Chinook to film their annual Man Overboard drills and re certification. It was a pleasure to watch the crews working together to demonstrate how fast they can save a life. The St. Andrew’s Bay Pilots are deeply saddened to announce the passing of Captain Phillip Brady, age 46, on January 12, 2026.

The RCMP are investigating the death of a man who went overboard outside St. John's harbour from a pilot boat early Tuesday morning.

U.S. Coastgard has published an update of their Recommendation for Pilot Transfer Arrangements in latest Marine Safety Information Bulletins (MSIB).

"Recent deaths of maritime pilots while embarking commercial vessels highlight the risks of operating in an
unforgiving maritime environment. To ensure the safety of all personnel boarding a vessel at sea, the Coast
Guard reminds vessel owners and operators of the requirements contained in the Safety of Life at Sea (SOLAS) Chapter V, Regulation 23 and strongly recommends that owners and operators follow the recommendations within IMO Resolution A.1045(27) – Pilot Transfer Arrangements. "

The pilot at Nagasaki Port fell into the water at around 0530 Tokyo time on 5 May while boarding the cruise ship DIAMOND PRINCESS, which was about to enter Nagasaki.
