Today i tagged along on the pilot boat Pacific Pathfinder and her sister ship the Pacific Chinook to film their annual Man Overboard drills and re certification. According to an article from "Safety at sea" the Korean Investigation Authority has published the first report about accident of container vessel "Milano Bridge" on April 6th 2020 in Busan.

According to AIS past track data, the vessel was obviously too fast on 9 knots and also going down the wind (4-5 bft., take a look at the exhaust from the stack) when entered the inner harbour considering the size and displacement. That speed was approximate 3 ship lengths to the pier and there was the on pier wind after the turn. Why the ship entered the port so fast will be the subject of the investigations to be awaited. The pilot at Nagasaki Port fell into the water at around 0530 Tokyo time on 5 May while boarding the cruise ship DIAMOND PRINCESS, which was about to enter Nagasaki.

Jacobsen Pilot Service and Los Angeles Pilot Station published a notice to all ships to inspect pilot ladders before the pilot boards.

Reason is an incident happened at the port of long beach in July 2020. Pilot ladder rope broke while the pilot was on the ladder - luckily the pilot was able to hold on.
