Today i tagged along on the pilot boat Pacific Pathfinder and her sister ship the Pacific Chinook to film their annual Man Overboard drills and re certification. Dredger OCEANLINE 5001 sailing at full speed struck anchored tanker STROVOLOS with moored alongside supply vessel, at around 1140 LT (UTC +8) Oct 2, on Melaka anchorage, Malaysia, Malacca Strait. The results can be seen on video – tanker suffered portside hull breach above waterline and apparently, some damage on cargo deck.

Manoeuvring large ships at close quarters and on shallow water is one of the most difficult aspects of shiphandling because of the complex hydraulic interactions depending on the ships' speeds, on the water depth and on lateral restrictions like in canals. Training is conducted both on meeting and on overtaking ships in shallow waters. This video shows how overtaking in a canal can easily fail.

The U.S. National Transportation Safety Board (NTSB) has released a Marine Accident Brief about an accident that occurred in March 2019, involving the towing vessel Dixie Vandal, noting that the BNWAS alarm didn't prevent the fatigued pilot from falling asleep.

In it´s August 2022 monthly safety scenario, the Swedish Club describes an accident where a pilot had fallen into the water, after one of the side ropes of the ladder broke.

Eray Demirkol, one of the pilots of the Dardanelles Strait General Directorate of Coastal Safety, fell from the pilot ladder into the sea while he was boarding the 176-meter bulk carrier 28653 DWT named "Lord Nelson".
