Going down the accommodation ladder with neither lifevest nor safety line must have been based on similar, past, successful operations, which may have never been challenged on board. This ‘slippage’ in safety is a pernicious and common phenomenon.
The “DangerousLadder” page on facebook is nowadays a very well known community in the maritime world. It is steadily growing towards 3000 members, which will be the next milestone membership-wise.
On 9 February 2019, near the Port of Hamburg, the EVER GIVEN struck the 25-metre HADAG ferry Finkenwerder, moored at the Blankenese jetty, and severely damaged it. Two minutes after the collision, a ban was imposed on sailing on the Elbe due to strong winds.

AMSA: A pilot was seriously injured after losing balance when a large wave impacted the vessel at the same time as they stood up to remove their lifejacket while in the cabin.
With only 10 days of preparation time, NAUTITEC successfully executed a high-end simulation of the Odyssey`s river-passage on a scientific level and they are glad to have seen simulation become reality once again.
