According to the International Maritime Pilots Association (IMPA), the ship “Grand Radiant” suffered an incident where the accommodation ladder entire outboard side railing fell away due to a pin becoming loose.

An investigation is underway following a collision between "Florence Spirit" and "Alanis" in the Welland Canal. A Japanese bulk carrier that has spilled more than 1,000 tons of oil since running aground off the coast of Mauritius has broken in two. Officials said the split was caused by a crack in a cargo hold, after the ship's condition deteriorated severely overnight. Emergency teams and thousands of volunteers have been racing to siphon off remaining fuel on board the ship.

Apparently there were two crew members on board, but no pilot. One of crew was recovered and declared dead, another one, skipper of the boat, is missing.

There are many things in both our everyday and professional lives which we take for granted and never question the origins of, an example of this might be the IMPA recommended ‘pilot mark or pilot line’, which is sometimes seen on the side of vessels indicating where a vessels freeboard exceeds 9 metres.
