As a rule, every pilot has the right to refuse an unsafe ladder. Safety first! A brave pilot is not the one who uses an unsafe ladder, but the one who, in case of doubt, asks for a different ladder or a different setting.
Unfortunately, too many pilots climb a dubious ladder when in doubt.

A 35-year-old male sea pilot of the Transnet National Ports Authority (TNPA) fell into the sea from a crude oil tanker near the N Shed Wharf in the port of Durban. A rope of the pilot ladder is said to have snapped for an undetermined reason when the man disembarked while leaving the crude oil tanker which was leaving the port of Durban.

Pilot transferring is unsafe and dangerous by Harbor Tug Boat from or to the ship. I analyzed and observed at many ports, Pilots using Harbor Tug Boat for transferring. Due to many incidents involving pilot ladders and man ropes in South Australian ports, and in effort to improve pilot safety, from 1 January 2022 Flinders Ports will require visiting vessels and their Masters to take additional precautions
