A Port Otago deckhand was seriously injured in a workplace fall yesterday afternoon.

Port Otago chief executive Kevin Winders said the deckhand on a pilot boat was preparing a ladder to allow a maritime pilot to climb aboard an oil tanker.

Product tanker NORTHSEA RATIONAL in load contacted embankment in Neumuelen, Hamburg, in the morning Nov 25, while maneuvering to berth, according to FleetMon.
It stated: “Tanker sustained serious damages to starboard bow, cause of accident yet unknown.
“She was berthed after accident, and as of evening Nov 25, remained, berthed. There was no hull breach in cargo tanks area, no cargo loss reported.”

The video was found on some social media channels in June 2020. When so many elementary mistakes are made and so many risks are taken, this is exactly what can happen! It's a demonstrative example of an unprofessional action: 1) Where is the life jacket? 2) No backpack on the shoulders. Use a rope to have the backpack lifted by the deck crew. 3) Where is the rest of the crew (on vessel / on the small boat) for safe assistance? What other mistakes have you discovered? What happened?
MV "Shahraz" and MV "Samudra Sakti I" had run aground to the shallow Batu Berhanti in the Indonesian territorial waters within 6 minutes, which is surveyed with minimum water depths of 8.3 m in the northern part and up to 0.5 m in the southern part . It is located south of TSS Singapore Strait’s eastbound traffic lane.
A chain of events which must be described as extra ordinary rarity....
