Ships losing power is not uncommon, Columbia River bar pilot says

River Mersey Pilot Boat going slowly inbound on the river before turning and heading northbound again.

Pilot boat with 5 people on board collided with breakwater at Kobe Port, Japan, at around 0300 Tokyo time Sep 4. 52-year old skipper of the boat, and 71-year old pilot, died in the collision, 3 crew were seriously injured.

A bulk carrier was departing port with a pilot on the bridge to assist with the undocking and manoeuvre downriver. The operation was also assisted by two conventionally propelled tugs, one forward and one aft.

To monitor the vessel’s progress, the pilot had set up his portable pilotage unit (PPU). He had connected the rate-of-turn generator to the vessel’s pilot plug and had set a variable range marker on the radar with a radius of 0.5 nm.

The LNG carrier ‘Genesis River’ collided with a 297-foot-long tank barge being pushed ahead by the 69-foot-long towing vessel ‘Voyager’, spilling petrochemical cargo into the waterway and capsizing a barge.

It was a near-miss Thursday morning at Port Tampa Bay as a harbor pilot managed to stop a cargo ship from crashing. The ship experienced a mechanical failure as it pulled into port. Between 2005 and 2018, the TSB received reports of 26 girding situations resulting in 21 capsizings. Girding occurs when a vessel is pulled broadside by a towline force and is unable to manoeuver out of this position. The TSB created this video to illustrate the factors leading to girding and the recovery methods.
