An ATSB interim report outlines the sequence of events leading to the collision between the container vessel Maersk Shekou and the berthed tall ship STS Leeuwin II in Fremantle on 30 August 2024.

The Ministry of Transport can confirm that the pilot boat, St David, was involved in a crash with a reef off of the Northeast Breaker Beacon on Thursday, May 6th, 2021 at 6:48am.

Dredger OCEANLINE 5001 sailing at full speed struck anchored tanker STROVOLOS with moored alongside supply vessel, at around 1140 LT (UTC +8) Oct 2, on Melaka anchorage, Malaysia, Malacca Strait. The results can be seen on video – tanker suffered portside hull breach above waterline and apparently, some damage on cargo deck.

According to the International Maritime Pilots Association (IMPA), the ship “Grand Radiant” suffered an incident where the accommodation ladder entire outboard side railing fell away due to a pin becoming loose.

Another accident was reported on YouTube on Feb. 3rd 2020 by akif pradias. "This video shown how's hwalein pilot got accident while boarding the vessel, its teach us to be more carefully during pilot transfer..." We do not put videos of accidents on our website out of voyeurism. We would like to point out that the work of a pilot is always dangerous, especially when embarking and disembarking! When you are a Marine Pilot at work, hoping and praying that the ladders which let you embark the vessel are stable, safe and not dangerous.
