Going down the accommodation ladder with neither lifevest nor safety line must have been based on similar, past, successful operations, which may have never been challenged on board. This ‘slippage’ in safety is a pernicious and common phenomenon.

The hazards of Pilot boarding
Throughout 2016, the International Maritime Pilots’ Association (IMPA) held a safety campaign focused upon the standard of pilot ladders and associated equipment. CHIRP supported this campaign and received many reports on the subject.

Dredger OCEANLINE 5001 sailing at full speed struck anchored tanker STROVOLOS with moored alongside supply vessel, at around 1140 LT (UTC +8) Oct 2, on Melaka anchorage, Malaysia, Malacca Strait. This video shows an introduction on the Spiral-Test-Manoeuvre and describes the procedure how to steer through this manoeuvre and measure the data needed. The results will be displayed in a characteristic diagram allowing to identify stable and unstable conditions and related, detailed information, e.g. spiral loop width and height, rudder angle of neutral effect. Samples of stable and unstable ship behaviour demonstrations will be shown by Fast Time Simulation using the SAMMON Planning tool.
