Today i tagged along on the pilot boat Pacific Pathfinder and her sister ship the Pacific Chinook to film their annual Man Overboard drills and re certification. According to AIS past track data, the vessel was obviously too fast on 9 knots and also going down the wind (4-5 bft., take a look at the exhaust from the stack) when entered the inner harbour considering the size and displacement. That speed was approximate 3 ship lengths to the pier and there was the on pier wind after the turn. Why the ship entered the port so fast will be the subject of the investigations to be awaited. U.S. Coastgard has published an update of their Recommendation for Pilot Transfer Arrangements in latest Marine Safety Information Bulletins (MSIB).

"Recent deaths of maritime pilots while embarking commercial vessels highlight the risks of operating in an
unforgiving maritime environment. To ensure the safety of all personnel boarding a vessel at sea, the Coast
Guard reminds vessel owners and operators of the requirements contained in the Safety of Life at Sea (SOLAS) Chapter V, Regulation 23 and strongly recommends that owners and operators follow the recommendations within IMO Resolution A.1045(27) – Pilot Transfer Arrangements. "

Pilot Training requirements were reviewed afer the ship hit the seabed amd tugs damaged in Bluff Harbor.
South Port in Bluff has accepted the Transport Accident Investigation Commission's recommendation and reviewed its systems.
