Today i tagged along on the pilot boat Pacific Pathfinder and her sister ship the Pacific Chinook to film their annual Man Overboard drills and re certification. Jacobsen Pilot Service and Los Angeles Pilot Station published a notice to all ships to inspect pilot ladders before the pilot boards.

Reason is an incident happened at the port of long beach in July 2020. Pilot ladder rope broke while the pilot was on the ladder - luckily the pilot was able to hold on.

What happened?
MV "Shahraz" and MV "Samudra Sakti I" had run aground to the shallow Batu Berhanti in the Indonesian territorial waters within 6 minutes, which is surveyed with minimum water depths of 8.3 m in the northern part and up to 0.5 m in the southern part . It is located south of TSS Singapore Strait’s eastbound traffic lane.
A chain of events which must be described as extra ordinary rarity....
