AMSA has published the first marine incident annual report
AMS has released the annual report of marine incidents reported from regulated Australian and foreign flagged vessels in Australian waters. It includes an analysis of reported marine incidents during 2016 to 2019, with a focus on 2019 data.

The 2019 annual report presents an analysis of marine incidents including fatalities reported to AMSA. It provides an insight into reported marine incident data between 2016 and 2019 in Australian waters. The report aims to provide an overview of current trends over the last 4 years in maritime safety.

Marine incident reporting helps inform proactive and reactive approaches to managing safety. The information from these marine incident reports is used reactively by implementing control measures on a case-by-case basis, and proactively by analysing trends and implementing more systemic measures.

On August 5, 2020 at approximately 10:30 pm, Sandy Hook Pilot Captain Timothy M. Murray was involved in an incident while boarding a tanker vessel inbound to the Port of New York & New Jersey. He sustained injuries after falling from a pilot ladder and was evacuated to a local hospital where his injuries proved to be fatal.

Manoeuvring large ships at close quarters and on shallow water is one of the most difficult aspects of shiphandling because of the complex hydraulic interactions depending on the ships' speeds, on the water depth and on lateral restrictions like in canals. Training is conducted both on meeting and on overtaking ships in shallow waters. This video shows how overtaking in a canal can easily fail.

Jacobsen Pilot Service and Los Angeles Pilot Station published a notice to all ships to inspect pilot ladders before the pilot boards.

Reason is an incident happened at the port of long beach in July 2020. Pilot ladder rope broke while the pilot was on the ladder - luckily the pilot was able to hold on.
