The theory of the 'Swiss cheese' model of incident causation and investigation.

Back in August 2022, we received an offer to accompany the Port Colborne Pilot boat to make a video about the process of putting Pilots on board ships sailing on the Great Lakes. Port Colborne is the upbound terminus of the Welland Canal, and opens onto Lake Erie.

The Canadian Hydrographic Service (CHS) wishes to promote the implementation of the new S-100 standards to provide mariners with access to additional information (bathymetry, surface currents and water levels) and greater versatility in the display of these data.

IMPA has published examples of best practice together with a letter from Capt. Simon Pelletier, President of IMPA.

Captain Matt Glass, a deputy pilot with the Houston Pilots, boards and guides the UAL Capetown on the Houston Ship Channel from the landing area outside Galveston Bay to the Manchester docks in Houston, a voyage of about 7 hours.

The role of a Tug Master undertaking harbour towage activities is a very specific one. He/she has to, alone or in cooperation with other Tug Masters, assist a ship in a safe and efficient way using his/her tug to the best of its qualities. 

Maritime pilots work in an irregular deployment system (rotation system) with unpredictable work assignments under high levels of physical and mental stress. Fatigue or chronic diseases, e.g. coronary heart disease, peptic ulcers or gastritis can occur as a consequence.

Improvements to pilots’ situation awareness (SA) in maritime navigation are critical to reducing human errors, which have caused 75% to 96% of marine accidents over the last few years.

A good working relationship with the pilot, effective Master Pilot Information Exchange at the start of the pilotage followed by well performing Bridge Resource Management during the pilotage passage, are important factors in a successful pilotage.
