The IMPA safety campaign on pilot ladders has run for a long time now. In this document the results of the IMPA safety campaign have been analyzed from 2016 to 2020.

The pilot at Nagasaki Port fell into the water at around 0530 Tokyo time on 5 May while boarding the cruise ship DIAMOND PRINCESS, which was about to enter Nagasaki.

The Container Ship RDO Conception arriving in Prince Rupert after a 10 day voyage from Tokyo. One of the biggest ships in the world, 23,000+ TEU container ship MSC MIA, contacted gantry crane while leaving container terminal at Valencia, Spain, in the morning Sep 13. Crane collapsed, crane operator sustained injures and was hospitalized.

Editors note: A film of a pilot boat has just been published on YouTube with the title "How Dangerous being in a Pilot Boat..." on YouTube. I'm not sure if this really shows a dangerous maneuver, maybe the experts can comment on it here.
But in principle it is problematic to approach the ship in such a small Pilot Boat in the wake from astern and cross the waves there. I remember an accident from Finland in December 2017 where the boat capsized and people died. Is this situation shown here...
