Singapore strait accident: Iranian Container Ship MV SHAHRAZ broke into two

Bulk carrier COFCO 1 with 41,900 tons of soybean resumed sailing downstream after grounding, which took place on May 25-26, but at around 0630 UTC May 27 she ran aground again, this time in San Pedro area at 269 kilometer mark, Parana river.

What happened?
MV "Shahraz" and MV "Samudra Sakti I" had run aground to the shallow Batu Berhanti in the Indonesian territorial waters within 6 minutes, which is surveyed with minimum water depths of 8.3 m in the northern part and up to 0.5 m in the southern part . It is located south of TSS Singapore Strait’s eastbound traffic lane.
A chain of events which must be described as extra ordinary rarity....

To monitor the vessel’s progress, the pilot had set up his portable pilotage unit (PPU). He had connected the rate-of-turn generator to the vessel’s pilot plug and had set a variable range marker on the radar with a radius of 0.5 nm.

Apparently there were two crew members on board, but no pilot. One of crew was recovered and declared dead, another one, skipper of the boat, is missing.

State regulators on Friday banned cargo ship pilots from using personal cellphones on Maryland waters. The NTSB determined that the probable cause of the contact of the Miss Mollye D tow with the Route 182 bridge was a loss of control of the tow by the pilot at the helm of the towing vessel, likely due to impairment by factors such as fatigue or drug use.
