Singapore strait accident: Iranian Container Ship MV SHAHRAZ broke into two

Bulk carrier COFCO 1 with 41,900 tons of soybean resumed sailing downstream after grounding, which took place on May 25-26, but at around 0630 UTC May 27 she ran aground again, this time in San Pedro area at 269 kilometer mark, Parana river.

Safe Passage in The Singapore Strait is a training package of two videos and an Interactive computer-based learning module produced by the maritime authorities of Indonesia, Malaysia and Singapore. What happened?
MV "Shahraz" and MV "Samudra Sakti I" had run aground to the shallow Batu Berhanti in the Indonesian territorial waters within 6 minutes, which is surveyed with minimum water depths of 8.3 m in the northern part and up to 0.5 m in the southern part . It is located south of TSS Singapore Strait’s eastbound traffic lane.
A chain of events which must be described as extra ordinary rarity....

According to the International Maritime Pilots Association (IMPA), the ship “Grand Radiant” suffered an incident where the accommodation ladder entire outboard side railing fell away due to a pin becoming loose.

A pilot of Sandy Hook, Dennis Sherwood fell off a ladder while boarding a ship today at 4:30 a.m. and died of his serious injury in Staten Island hospital.

AMSA notes with concern that since November 2017 several pilots’ lives have been placed at risk, in six separate incidents where man ropes have parted, or its securing point has failed. In addition, AMSA regularly receives reports and complaints about non-compliant pilot transfer arrangements
