Singapore strait accident: Iranian Container Ship MV SHAHRAZ broke into two

Container ship SHAHRAZ and bulk carrier SAMUDRA SAKTI I are reported to run aground in Singapore Strait south of St John Island at around 1900 UTC May 10, close to each other, while proceeding in the same direction, probably trying to avoid collision. As of 0700 UTC May 11, both ships remain in the same positions, coordinates don’t change. SHAHRAZ is en route from Port Klang to Yangshan China, SAMUDRA SAKTI I is en route from Belawan to Bayah, southwest Java.

What happened?
MV "Shahraz" and MV "Samudra Sakti I" had run aground to the shallow Batu Berhanti in the Indonesian territorial waters within 6 minutes, which is surveyed with minimum water depths of 8.3 m in the northern part and up to 0.5 m in the southern part . It is located south of TSS Singapore Strait’s eastbound traffic lane.
A chain of events which must be described as extra ordinary rarity....

Video Refusing to board due to a pilot ladder not rigged correctly

This lady came into port in an excellent day, however when I inspected the pilot ladder prior to board I saw that it was rigged using D-Shackles fastened between the side ropes, transferring all the weight and load of the ladder to the clamps and chokes. Truly an unsuitable method of securing the ladder. Retrieval line was also not above the first spreader
After pulling out the pilot boat, the ship was contacted by VHF and the captain was asked to instruct the crew in the usage of ropes and...
