Dredger OCEANLINE 5001 sailing at full speed struck anchored tanker STROVOLOS with moored alongside supply vessel, at around 1140 LT (UTC +8) Oct 2, on Melaka anchorage, Malaysia, Malacca Strait. Reason is an incident happened at the port of long beach in July 2020. Pilot ladder rope broke while the pilot was on the ladder - luckily the pilot was able to hold on.

In it´s August 2022 monthly safety scenario, the Swedish Club describes an accident where a pilot had fallen into the water, after one of the side ropes of the ladder broke.

On August 5, 2020 at approximately 10:30 pm, Sandy Hook Pilot Captain Timothy M. Murray was involved in an incident while boarding a tanker vessel inbound to the Port of New York & New Jersey. He sustained injuries after falling from a pilot ladder and was evacuated to a local hospital where his injuries proved to be fatal.

According to various reports, the motor vessel "Else" damaged the gate of the Kiel-Holtenau lock at 5:00 a.m. on 29 August when it entered the closed gate without a pilot.
