Dredger OCEANLINE 5001 sailing at full speed struck anchored tanker STROVOLOS with moored alongside supply vessel, at around 1140 LT (UTC +8) Oct 2, on Melaka anchorage, Malaysia, Malacca Strait. The UK Marine Accident Investigation Branch’s (MAIB) has published their latest annual report showing that pilot ladders remain a safety issue for workers.

Norwegian coast guard said that there is still a risk that the ship may capsize and pose an environmental
hazard.The ship is still drifting, and is located 40-50 nautical miles west of Alesund. KV Sortland is still on
site, and they are the Coastal Administration's eyes and ears.

On March 15, 2021, about 1302 local time, the oil tanker Riverside with a crew of 21 and 2 pilots was transiting outbound from the port of Corpus Christi, near Ingleside, Texas, in a loaded condition when the vessel lost propulsion and struck the no. 4 loading dock at the Moda Ingleside Energy Center. 1 No pollution or injuries were reported. Damage to the vessel was estimated at $550,000. The estimated property damage to the facility was $7 million.

Despite clear regulations and regular maintenance requirements, safety often falls by the wayside, as the current example of a bulk carrier vividly demonstrates.

The Auckland District Court has fined the Ports of Auckland NZD424,000 (USD300,500) for exempting its pilot boats from speed limits in Waitemata Harbour after a swimmer was struck and killed (April 2017).
