Cargo ship crushes tugboat in Panama Canal

It was a near-miss Thursday morning at Port Tampa Bay as a harbor pilot managed to stop a cargo ship from crashing. The ship experienced a mechanical failure as it pulled into port. Pilot boat with 5 people on board collided with breakwater at Kobe Port, Japan, at around 0300 Tokyo time Sep 4. 52-year old skipper of the boat, and 71-year old pilot, died in the collision, 3 crew were seriously injured.

A pilot boat caught fire near the entrance of the Port of Recife, on Saturday morning (15 January). The fire would have started around 11h. In a note, the port authority explained that the two people who were in the pilot boat were rescued by another pilot boat "Serrambi".
The causes of the accident had not yet been identified, but would be investigated.

The US Coast Guard has now issued "Marine Safety Information Bulletin 21-20" which repeats the recommendation for pilot transfer agreements.
