On 30 March 2023, a fatal accident occurred in the North Sea near IJmuiden during a pilot transfer from the pilot boat to a seagoing vessel.

Built by Luke Powell and his working Sail Yard in Truro, Cornwall, launched in February 2020. Throughout 2016, the International Maritime Pilots’ Association (IMPA) held a safety campaign focused upon the standard of pilot ladders and associated equipment. CHIRP supported this campaign and received many reports on the subject.
